Leaving the hospital and returning home can be a challenging transition for anyone, filled with mixed emotions of relief and apprehension. At Claremont Living, we understand these feelings and are here to support you every step of the way. Our dedicated service is designed to assist individuals in smoothly adjusting back to their home environment after a hospital discharge. We offer shopping support, befriending services, assistance with applying for various services, access to healthcare, and connections to local resources.
Our compassionate team is committed to making your recovery journey as comfortable and stress-free as possible, ensuring you have the support and resources you need in the comfort of your own home.
What does this service offer?

Food parcels that meet dietary requirements and religious dietary requirements, coordinated with your social worker to cater to these needs, provided with 6 weeks of continuous support, once a week.

Befriending services to offer companionship and support if you feel lonely.

Assistance with applying for services such as Personal Independence Payment (PIP), benefits, and Universal Credit.

Setup for online shopping to allow you to order groceries directly to your home.

Access to healthcare by collecting medications for you within a 1-hour timeslot.

Help with applying for a Blue Badge and bus pass.

Assistance with arranging transport to access local services such as church or social clubs.

Signposting to external services such as cleaning if needed, though personal care is not provided.
